Low back pain is probably the most common pain to afflict the human race. During the lifetime of anyone of us, this is likely to occur on certainly more than one occasion. The human frame consists of a skeletal framework that we call the spine, In turn the spine is made up of 24 individual bones or vertebrae and between each vertebra is a disc which acts as a shock absorber.
Without these discs the bones would quickly wear out and fracture from everyday forces such as jumping off the back of a truck. Between the discs are the spinal nerve roots which in sprout off from off the spinal cord. The spinal cord is a continuation of the brain and commences at the base of the skull and continues right down to the “tail bone”.
There are seven layers of muscles which support the spine and help keep us upright. Now that we have had this mini-lesson of anatomy, we can get some idea of just how complex the musculoskeletal system is. No wonder that low back pain occurs so frequently when we are so reliant on all of the above systems to keep us straight and active.
The spinal nerve roots are incredibly sensitive to changes of posture and position and register pain if they are in way squeezed or impinged. While we prefer not to actually call it a pinched nerve, the pain that occurs certainly feels as if the nerve is having just that done to it. In addition the vertebrae of the low back, which we call lumbar vertebrae, have small facet joints which also have nerve endings and are sensitive to pain.
When irritation occurs, either directly from spinal nerves being impinged or from facet joints being fixated (subluxated), pain occurs in the low back. The muscles of the spine which were referred to previously, receive their nerve supply from the same nerves that exit from the spinal cord, between the vertebrae.
These muscles contract and go into painful spasm when their nerve supply is compromised. They do this as a reflex action to protect the affected area and the result can be very painful. The sciatic nerve is a large nerve which is made up of five individual spinal nerves of the low back. The sciatic nerve passes through the buttock (gluteal) region and descends down both legs. There is a sciatic nerve on each side of the body.
The term “sciatica” refers to the very painful condition which occurs when one or more of the five nerve roots become impinged. Both pain and numbness or tingling of the nerves can take place from the gluteal region, right down to the legs, feet and toes. In addition, as I have already mentioned, the muscles receive their nerve innervation from those very same nerves
Those muscles can become weak and in some cases, atrophied. This means that a leg muscle will become thinner and weaker on one side when compared to the other leg muscle. Referred pain has now occurred with the initial point of nerve pressure at the lumbar spinal region now having a direct effect on a distant area, such as the toes and feet.
As a chiropractor of some twenty five years experience, I advise my patients who present with this problem to have x rays and possibly an MRI or CT scan. This will then form the basis of my orthopedic and neurological examination after which an accurate diagnosis can be made. If appropriate, chiropractic treatment can then be commenced and the patient started on the road to recovery.
In addition to the chiropractic treatment which might take place twice weekly over some weeks, the patient will be instructed how to perform exercises to strengthen both back muscles and the intrinsic core muscles of the body. This will ensure upright posture and spinal stability. There is absolutely no need for a person to put up with low back pain for the rest of their life when gentle and effective chiropractic care is readily available.

The hip area is a strictly defined region. In anatomy, the hip is the bony projection of the femur which is known as the greater trochanter, and the overlying muscle and fat. The hip joint, scientifically referred to as the acetabulofemoral joint, is the joint between the femur and acetabulum of the pelvis and its primary function is to support the weight of the body in both static (e.g. standing) and dynamic (e.g. walking or running) postures.
In my chiropractic practice I often see patients who tell me that they have pain in the hip. On examination they may have pain in the low back region, in the buttocks or in the sacroiliac joint. It would seem that hip pain can be difficult to describe and difficult to pin down to a specific problem.
One of the first things that I do in these cases is to ask the patient to have their low back x-rayed, This will include both hip joints. Other tests will be orthopedic, neurological and range of motion tests to see exactly which movements are both restricted and painful.
Let us look at some causes of hip pain.
1. Hip bursitis. This means inflammation of the bursa over the outside of the hip joint.
2. Osteoarthitis. This means wear and tear or degenerative joint disease.
3. Hip fracture. This means a broken bone.
4. Osteomyelitis. This means an infection in the bone itself.
5. Osteonecrosis. This means a restricted blood supply to the bone is causing the bone to break down and die.
Any of the above conditions require medical treatment and are not within the province of a chiropractor to deal with.
When do you need to call your doctor about your hip pain? If you are unsure of the cause of your symptoms, or if you do not know the specific treatment recommendations for your condition, you should seek medical attention. Treatment of hip pain must be directed at the specific cause of your problem. Some signs that you should be seen by a doctor include
* Inability to walk comfortably on the affected side
* Injury that causes deformity around the joint
* Hip pain that occurs at night or while resting
* Hip pain that persists beyond a few days
* Inability to bend the hip
* Swelling of the hip or the thigh area
* Signs of an infection, including fever, redness, warmth

Step 1 – Location
The first step is to decide the location. “Where does it hurt?”
1. Axial lower back pain: This lower back pain hurts only in the low back. Pain does not travel into any other area.
2. Radicular lower back pain: This lower back pain hurts in the low back, and also radiates down the backs of the thighs into one or both legs.
3. Lower back pain with referred pain: Diagnose lower back pain with referred pain if it hurts in the low back area, and tends to radiate into the groin, buttocks, and upper thighs. The pain will rarely radiate below the knee, but may seem to move around.
Step 2 – Symptoms
Once you diagnose lower back pain as to location, you will consider symptoms. “How does it feel?”
1. Worsens with certain activities: If you play football, for example, the pain is worse.
2. Worsens in certain positions: Perhaps it gets worse if you stand for too long. Or it is more painful after you sit in a car.
3. Feels better after rest: Resting from the activity or position usually reduces the lower back pain.
4. Deep and steady: Not a sharp muscle catch, this pain is constant and deep within the affected areas.
5. Severe: The pain is excruciating, possibly more so in the calf than the lower back.
6. Numbness and tingling: There may be “pins and needles” within the area.
7. Fleeting pain: Pain may seem to come and go, leaving you unsure at times just how it feels.
8. Achy and dull: Like the flu, this pain is sore and dull, though sometimes intensifying.
9. Migratory: It hurts in one spot, then another.
Diagnosis
AXIAL: If location is best described by number 1 above, and symptoms are a combination of 1, 2, and 3, you can probably diagnose lower back pain as being axial – the most common type. This is also called “mechanical” lower back pain. A variety of back structures can cause axial lower back pain, and it is difficult to identify which is the cause. Axial pain gets better on its own, and about 90% of patients recover within six weeks.
RADICULAR: If location is best described by number 2 above, and symptoms are a combination of 4, 5, and 6, you can probably diagnose lower back pain as being radicular – commonly called sciatica. This lower back pain is caused by compression of a lower spinal nerve, usually the sciatica nerve that runs from the spinal column, down the back of the thighs to the feet. Doctors usually recommend conservative treatment such as physical therapy exercises, medications, and possibly spinal injections, for six to eight weeks.
REFERRED: If location is best described by number 3 above, and symptoms are a combination of 7, 8, and 9, you can probably diagnose your pain as being lower back pain with referred pain – the least common type. This lower back pain is treated the same as axial back pain and frequently goes away as the problem resolves on its own.
How do you diagnose lower back pain?
Diagnose lower back pain with care. You need an accurate diagnosis, which your physician can best make, to be sure no underlying causes need attention. It is not enough to know you have sciatica. You need to know the underlying cause of the sciatica to determine treatment options.

Your posture may also suffer as the weight is concentrated in your uterus, changing your center of gravity. This also will put more pressure and stress on your hips.
Sleeping with a pillow between your legs at night will help take some of the pressure off of your hips. There are also body pillows available that are curved to support not only your hips, but also your stomach and neck. By supporting these specific areas, your spine will also be supported, which will help decrease the pain during the day as well. Using a large exercise ball to sit on can also help take the stress off of your hips and pelvic area, but be sure to get a helping hand for balance when sitting down or getting up!
If it seems that your hip pain is more prominent at certain times of the day or during activities such as walking, bending at the hips, or standing for a long time, let your OB/GYN know. They may suggest seeing a physical therapist to help you ease the pain and gain more flexibility.
Though walking is often suggested to stay healthy and prepare yourself for giving birth, if a woman is having hip pain, she may be advised to limit her walking. Many women also enjoy yoga, pilates, or other gentle stretching exercises during pregnancy to help ease pain, improve their flexibility, and prepare their bodies for giving birth.
Chiropractic care may also be suggested. If your hips are out of alignment, you may benefit from an adjustment from a chiropractor who has been trained in pregnancy care. A massage may feel very good to a pregnant woman as well. Therapeutic massage is much more effective than relaxation massage when you have hip pains during pregnancy. You will want to make sure you are able to go to a therapeutic masseuse who is trained in helping pregnant women, because there are acupressure points in the legs and feet that are associated with the reproductive system and should be avoided during pregnancy.
Not every woman will experience hip pain during pregnancy, but when it occurs it can be very unnerving. These are supposed to be joyous months for a woman, and you should be able to focus on planning for your new arrival. If you opt for a cane, it is recommended to use proper technique by carrying the cane in the hand opposite the injured hip. Move it forward at the same time you step out with the injured hip, so you are bearing weight on both the good hip and the cane. Then step out with your “good” hip.
3. Go Polar
Ice is the first line of defense against hip pain. It is recommended to crush some ice and place it in a resealable storage bag, so it has a greater surface area. Lay a thin towel over your hip, then apply the ice at the site of pain for 15 minutes. If your pain is acute, caused by an injury, for example, you may need to reapply the ice as often as once every hour. But for chronic pain such as arthritis, once or twice a day may be enough.
4. Be Reasonable
Don’t place too many demands on an aching hip. In general, stay away form any activity that aggravates your pain. You may have to drive to work instead of walking or take the elevator instead of bounding up three flights of stairs at least for the time being.
5. Strike A Pose
Yoga can also enhance flexibility. It is recommended to adopt this simple technique. Sit on the floor and place the soles of your feet together. Pull your feet in, as close to the groin as comfortable. Then let gravity pull your knees toward the floor. If this position is uncomfortable at first, recommend to modify it slightly by bending only one leg at a time and keeping the other leg extended. Hold the pose for 15 seconds. Repeat three times.
6. Walk In Water
To keep your hip limber while you recover, head to your nearest pool for an aquatic workout. Water takes the weight off your hip and allows you to do more without pain than you could on dry land.
7. Take A Tablet
An over-the-counter pain reliever such as ibuprofen or acetaminophen can help a hurting hip. But if the dosage recommended on the label does not do the trick, alert your doctor. Something more serious may be going on.

The 10 videos in this series will give you a complete overview of how, in 6 months, I eliminated the intense back pain that I lived with for 8 years. I purchased several inexpensive pieces of equipment, such as this Pilates Reformer, to make my exercise routine fun and effective, but they aren’t necessary. What is necessary is your commitment to a daily exercise routine.

Having suffered from hip pain myself, I know how frustrating and painful it can be, but did you know that your diagnosis might be incorrect and that it may be possible for you to heal yourself? Chronic hip pain is typically diagnosed as one of three possible syndromes: tendonalgia (also known as tendonitis), arthritis or bursitis, all of which may be misdiagnoses for the actual origin of your pain.
Arthritis – arthritis can be either rheumatoid arthritis or osteoarthritis. Many people with hip pain are given a diagnosis of osteoarthritis (wear and tear on the joint, usually seen on an x-ray) and assume that they will be dealing with anti-inflammatory medication and pain for the rest of their lives. Yes, you may show some moderate wear in your hip joint, but the pain may in fact be caused by tension myositis syndrome (TMS).
Bursitis – a bursa is a structure that protects underlying bone in areas of high pressure. Bursitis can affect the shoulder (subacromial bursitis,) or the hip, (trochanteric bursitis.) But in a lot of patients bursitis is a misdiagnosis and the real cause is TMS related tendonitis.
Tendonitis – is a group of disorders attributed to a painful tendon inflamed due to overuse. In certain clinics, it has been found that tendonitis is caused not by overuse, but by a subconscious pain creating mechanism of our own minds called tension myositis syndrome (TMS).
TMS, or tension myositis syndrome, is a painful condition caused by our subconscious mind. It is mistakenly diagnosed by doctors as many other conditions, including; bursitis, osteoarthritis, tendonitis, pinched nerves, eczema, and ruptured discs, just to name a few. The pain caused by TMS is real, but the reason for the pain is not due to a physical abnormality and therefore can be easily healed. TMS is used by our subconscious to divert our attention from the even more painful emotions of anger and resentment.
Since it is objectionable in most cultures to express anger and resentment we condition ourselves to hold it in. The amount of energy associated with negative emotions is quite great and your mind’s way of dealing with this repressed negative energy is to ‘put’ it somewhere in your body, such as your hip, shoulder, low back, or neck. Your subconscious mind then creates a painful condition, such as tendonitis, by restricting the oxygen and blood flow to the area.
TMS is little known in most medical circles, so asking your doctor about it will most likely do you little good. Physical Therapy for the Lower Back: How to Prevent and Treat Lower Back Pain
Lower back pain is one of the most common causes of job-related disability and why some people miss work. It is also the second most common neurological ailment in the United States, second only to headache. In fact, approximately 80% of adults in Western countries have, at some point, experienced lower back pain.
For some fortunate people, pain in the lower back may be resolved by itself or with the aid of medication within two to four weeks. However, there are some cases of lower back pain that may last for more than a few weeks, during which case the condition is termed as “chronic” and “progressive,” meaning it can only grow worse over time.
Moreover, 60-80% of those patients who suffer their first episode of lower back pain may experience recurring pain within one year.
According to current research, there are certain muscles in the back that work to stabilize the spine. When the spine or the back suffers an injury, these muscles are reflexively inhibited or shutdown. Worse still, these muscles do not spontaneously recover, and this is true even if patients do not feel pain and are able to return to normal activity levels.
As a result of the inhibition of these muscles, called lumbar multifidi and the transversus abdominus, lower back pain occurs. However, there are steps you can take to prevent the same thing from happening to you.
Lower Back Pain and Physical Therapy
One way to prevent the inhibition of the lumbar multifidi and transversus abdominus is through a series of physical therapy exercises.
Designed to strengthen the muscles of the lower back and keep the spine healthy, these physical therapy exercises may range from back stabilization exercises to muscle strength development and several wide variety of techniques.
In addition, a physical therapist may also recommend such methods as heat therapy, ultrasound, massage, mobilization, and education about posture and body mechanics in order to prevent lower back pain from recurring.

However, physical therapists and other healthcare professionals have observed that lower back pain is often a result of strained back muscles and ligaments due to any of the following activities:
Improper posture
Heavy lifting
Sudden awkward movement
Muscle spasm
Stress
We could all be guilty of the above activities. We may not suffer any back pains now, but it is likely that as we get older and the degree of inhibition of the back muscles as a result of these activities increases, back pain becomes a very distinct possibility.
In addition to these common activities, lower back pain may also result from specific conditions, such as:
Herniated disk (when the disk material presses on a nerve)
Sciatica (when a herniated disk presses on the sciatic nerve. The condition causes sharp, shooting pain through the buttocks and the back of the leg.)
Spinal stenosis (when the space around the spinal cord and nerve roots becomes narrow. This is caused by arthritis and bone overgrowth, the pain resulting from when a nerve gets pinched in the narrow space.)
Spondylosis (a type of arthritis affecting the spine due to degenerative changes brought on by aging)
Spondylolisthesis (when one vertebra in the spinal column slips forward over another)
Lower Back Pain: TREATMENT
The treatment of lower back pain depends on several factors, including the specific type of lower back pain (whether it is chronic or acute) and the purported cause.
For instance, acute lower back pain is commonly treated with pain relieving drugs, such as analgesics, or some forms of exercises that can help relax the muscles.
On the other hand, chronic back pain or one that lasts for more than two weeks and is progressive may be caused by some underlying condition, during which case the treatment plan may consist of resolving the underlying condition to treat the back pain.
Lower Back Pain and Physical Therapy Exercise
Physical therapy exercise is one of the most common methods of treating lower back pain. In fact, many home remedies for lower back pain consist of exercise, because the general theory is that if you remain active, you remain healthy. This is true in most cases.
However, for purposes of this article, the exercises featured here will be those that are practiced by physical therapists to treat patients with lower back pain.
Generally, in physical therapy exercises, the exercise program for back pain should encompass a set of stretching exercises, strengthening exercises, and low impact aerobics. Read below for more on these exercises:
-Stretching
The back of a person is composed of the spinal column and contiguous muscles, ligaments and tendons. All these are designed to move in consonance with each other so that any limitation in the range of motion in any of these components of the back result in back pain.
Stretching for lower back pain specifically targets soft tissues, such as muscles, ligaments and tendons, found in the back and around the spine. By stretching, the spine and soft tissues are mobilized, increasing motion and thus, relieving pain.
There are many kinds of stretching exercises employed by physical therapists. One is the Hamstring Stretching Exercise which works to relax tight hamstrings, a common symptom of lower back pain. This exercise is said to help decrease the intensity of lower back pain among sufferers.
-Strengthening
Physical therapists generally use two forms of strengthening and back pain relief exercises, usually depending on the specific condition of the patient. These are the McKenzie exercises and dynamic lumbar stabilization exercises. However, the two forms of strengthening exercises may also be combined should the therapist find it appropriate to do so.
-McKenzie Exercises
Named after a physical therapist in New Zealand, McKenzie exercises are primarily extension exercises that could help reduce pain generated from the disc space and also may help reduce the symptoms of herniated disc by reducing pressure on a nerve root.
For acute pain, the McKenzie exercises should be done frequently, at least once every two hours. In addition, patients are advised to avoid flexing their spine when exercising.
-Dynamic Lumbar Stabilization Exercises
Using this back exercise technique, the first thing that a physical therapist does is to look for the patient’s “neutral” spine. This refers to the position that allows the patient to feel the most comfortable.
Afterwards, when the patient is in that position, the back muscles are then exercised in order to “teach” the spine how to stay in this position.
Performing these exercises on a regular basis can help strengthen the back muscles and keep the spine well-positioned.
Low Impact Aerobic Exercises
The purpose of low impact aerobic exercise is to recondition the back. Patients who undergo reconditioning of the back through low impact aerobic exercise will have fewer episodes of lower back pain.
In addition, whenever an episode of lower back pain does occur, the pain is less intense and lasts only for a short period.
Another benefit of low impact aerobic exercise is that patients tend to stay functional that is, they can continue with their regular work and carry on with recreational activities. In contrast, patients who do not undergo low impact aerobic exercises typically experience the gradual loss of their functional abilities.
For low impact aerobic exercises to achieve their desired results, they should be continuous. This will increase the heart rate and keep it elevated as well as increase the production of endorphins, which are pain fighting hormones released by the body.
Here are some examples of low impact aerobic exercises that you may want to try in order to lessen or reduce lower back pain:
-Walking
One of the simplest forms of aerobic exercises, walking is generally considered as very gentle on the back. To get the maximum benefit from walking as a form of low impact aerobic exercise, walk two to three miles three times per week.
-Stationary Bicycling
This form of aerobic exercise is less painful on the back since there is lower impact produced. This is beneficial for patients with lower back pain who may find walking too painful.
